Can AI be Trusted in Healthcare? A Case Study

Artificial Intelligence (AI) is making big strides in healthcare, offering faster ways to diagnose illnesses, better patient outcomes, and valuable support for doctors’ decisions.

But as AI tools start to play a bigger role in healthcare, there are some real concerns around trust, ethics, and transparency. Can we rely on AI to understand the complexities of healthcare and not disrupt the relationship between patients and their doctors?

In this article, we dive into the trust issues surrounding healthcare AI by looking at a real-world case study.

We’ll break down what influences trust for both patients and doctors, explore how biases might creep into AI systems, and discuss the rules and regulations that are shaping this new area of healthcare.

What Makes AI Trustworthy?

For AI to be trustworthy in healthcare, it needs to demonstrate reliability, accuracy, and explainability.

According to a study , trust is a “psychological mechanism” essential for clinicians to rely on AI in complex, data-driven decisions.

However, unlike traditional software, AI’s outcomes can vary due to its evolving algorithms, which introduces a unique challenge for healthcare where consistency and predictability are crucial.

Three Main Dimensions of Trust : A Case Report

Three Main Dimensions of Trust : A Case Report

A Case Analysis observed that trust has three main dimensions—competence, integrity, and benevolence—when applied to healthcare providers.

Addressing these concerns is critical, as AI needs to maintain the same—or higher—standards expected from human clinicians.

The Patient Perspective: What Are Patients Concerned About?

A survey by Heath (2024) reveals that 44% of patients believe AI’s trustworthiness depends on its application, while another 43% are unaware of how AI is used in healthcare.

A generational divide in trust is also present, with Millennials showing the most openness to AI applications and Baby Boomers being more skeptical.

The lack of awareness and general wariness towards losing the “human touch” in healthcare are significant barriers to AI adoption.

Patient Concerns in Healthcare AI:

  • Loss of Human Connection: Patients worry AI could depersonalize their care, especially if used for diagnostic tasks.
  • Data Privacy and Security: Trust is jeopardized when patients feel uncertain about how their health data is managed.
  • Over-Reliance on AI: Concerns arise about clinicians overly relying on AI, potentially undermining the expertise of healthcare providers.

Trust Between Clinicians and A.I

AI’s ability to analyze massive datasets can offer valuable insights that are often inaccessible to humans.

However, clinicians are cautious about delegating decisions entirely to AI. One primary concern, as highlighted by Asan et al. (2020), is “calibrated trust.”

For effective AI integration, clinicians must maintain an optimal level of skepticism, avoiding blind trust while respecting AI’s analytical capabilities.

AI’s limitations—such as potential bias from unrepresentative data—can further affect the trust clinicians place in these tools. A study noted that clinicians’ trust might decrease in high-risk scenarios, especially when patient outcomes are at stake.

Another Case Study adds that clinicians’ trust in AI correlates strongly with AI’s perceived explainability and anthropomorphism.

AI with human-like features can generate higher trust, though it may also lead to overestimations of AI’s abilities, underscoring the need for clear understanding of AI’s actual capacities and limitations.

Regulatory Oversight

One crucial aspect of trust is regulatory oversight.

The FDA, for example, classifies AI software under “Software as a Medical Device” (SaMD), distinguishing between “locked” algorithms, which remain unchanged, and adaptive algorithms, which evolve with new data inputs.

Regulatory bodies, however, face challenges in setting standards for AI that self-adjusts, making the reliability of AI systems potentially inconsistent over time. This evolving nature demands continuous regulatory updates to ensure safety and performance.

The European Commission’s High-Level Expert Group (HLEG) has developed “Trustworthy AI” guidelines focused on human agency, robustness, transparency, and accountability.

These provide a framework for AI deployment but require adaptation for context-specific applications.

Moving Forward

Despite these challenges, patients and providers see the potential of AI in healthcare.

According to a Case Report , 52% of patients believe AI could be part of healthcare’s solutions, not its problems.

Clinicians also appreciate AI’s potential for reducing burnout by automating routine tasks, allowing more time for direct patient care.

Yet, to foster trust, developers and healthcare providers must prioritize transparency, regulatory compliance, and active patient involvement in AI implementation processes.

Key Recommendations for Trustworthy AI in Healthcare:

  • Enhanced Transparency: Explain AI processes and data use clearly to patients.
  • Fair and Inclusive Data: Ensure datasets are representative of diverse populations to mitigate bias.
  • Balanced Usage: Encourage AI as a support tool rather than a replacement for clinician expertise.
